Hackensack Meridian Health has developed the Five Pillars program – nutrition, activity, sleep, resilience and purpose – to help patients focus on their health and well-being goals. Each patient is given a survey to understand strengths and weaknesses in each of his or her pillars. Trained nurses go through the survey and offer support and guidance. The network’s efforts were featured along with models in other major systems, including the Veterans Health Administration.
