Fresh Match Program
Hackensack Meridian’s Fresh Match program aims to increase the purchase of fresh fruits and vegetables for New Jersey Supplemental Nutrition Assistance Program (NJ SNAP) recipients. Fresh Match will be available at grocery stores in seven counties in New Jersey.
Medication Program
Hackensack Meridian Healthy Connections Medications provides our uninsured and discharged patients with medications for chronic conditions at no cost. Patients who receive a prescription receive refills at their local Hackensack Meridian Health pharmacy with no out of pocket costs.
Medically Tailored Meals
Medically Tailored Meals Hackensack Meridian Healthy Connections Meals provides discharged, eligible patients with two medically tailored meals per day for up to one week in a reusable, insulated cooler bag. Medically tailored meals are reviewed by Hackensack Meridian’s medical team and provided to patients based on their medical and dietary needs.
