TEE Guidelines

Lucy Safi, D.O., FACC, FASE, Director of Interventional Echocardiography, Hackensack Meridian Hackensack University Medical Center, recently joined field experts as co-author in the guidelines paper to develop the American Society of Echocardiography’s new recommended standards for the performance of transesophageal echocardiographic (TEE) screening for structural heart interventions.

Published in the Journal of the American Society of Echocardiography, the new recommendations address the current American College of Cardiology and American Heart Association guidelines that suggest that echocardiographers serving on multidisciplinary teams caring for patients with structural heart disease should hold expertise in valve disease and transcatheter and surgical interventions, whether or not they are guiding procedures.

“This guidelines document is intended for the general cardiologist and provides a reference guideline for acquiring essential TEE images needed for structural heart disease assessment before intervention,” states Dr. Safi.

The recommendations include guidance for performing a standard 28-view comprehensive TEE examination with introduction of procedure specific structural imaging assessments. Included in the document are charts and example images of all recommended views for various structural heart procedures. The imaging protocols are intended to provide the required images needed to support a multidisciplinary team’s decision-making process. Included in the assessment will be to identify the mechanism of structural or valvular dysfunction, assess hemodynamics and anatomic severity of disease, and the specific anatomic features relevant to determining candidacy for intervention. Imaging approaches also are intended to facilitate procedural planning and intraprocedural imaging guidance.
