Geri Oncology Collaboration Enhances Overall Health of Older Patients

Coordinated Services and Support Promote Efficiencies and More Effective Care

The Division of Geriatric Medicine and John Theurer Cancer Center (JTCC) at Hackensack University Medical Center have launched a collaboration to provide enhanced geriatric-focused services to older patients receiving cancer care from JTCC.

The comprehensive approach leverages the shared electronic medical record to coordinate frailty assessment for older adults with a cancer diagnosis, psychosocial support, high-risk medications/polypharmacy management and end-of-life care support with the center’s hospice program. The program’s ultimate goal is to improve access to the most effective care for each patient and strengthen their overall health so they can better tolerate cancer treatments.

As cancer occurrence increases with age and the proportion of population that is reaching advanced age is continually increasing, the collaborative Hackensack University Medical Center program addresses a growing need to improve cancer care access and match each patient with the most effective treatments and support to ensure the best possible outcome. To address this need more broadly, the program also facilitates cross-disciplinary educational opportunities for geriatric fellows.
