About This Location
HMH - Palliative Care - Brick in Brick, NJ provides palliative care to help improve quality of life for people living with the physical symptoms and emotional stress of serious illnesses. We provide outpatient services that are commonly offered in a hospital or rehab setting.
Our CIPM team works closely with patients, families and caregivers to help manage physical symptoms, emotional stress and day-to-day challenges. We also help support communication between you and your care providers so you feel informed and supported throughout your care.
We are part of Hackensack Meridian Health, New Jersey’s largest health network, which gives our patients coordinated access to specialists and services across the system.
Additional LocationsWe have additional locations in Neptune, Manahawkin and Red Bank New Jersey.

- Help Manage and Control Symptoms: Pain, shortness of breath, difficulty sleeping, nausea and loss of appetite.
- Offer Resources and Education: Insurance assistance, financial issues, home care, support groups and transportation.
- Provide Emotional Support: Assist with feelings of anger, sadness, frustration, fear and loneliness.
- Advance Care Planning: Education and assistance with Advance Directives, Living Will and POLST.
- Encourage Quality of Life: Advocacy, involvement, communication and education.

Other indications for referral to palliative care for those patients with advanced illnesses include:
- Help with complex decision-making and determination of goals of care
- Unmet psychosocial or spiritual issues
- Support for family/caregivers
- Conflict regarding goals of care
- Frequent visits to the Emergency Department (more than once a month for the same diagnosis)
- More than one hospital admission for the same diagnosis within 30 days
- Prolonged length of stay (more than five days) without evidence of progress
- Prolonged stay in ICU setting without evidence of progress
