Hackensack University Medical Center Performs Its First Robotic Trans-Thoracic Rib Resection
Thoracic and vascular surgeon duo collaborate on precision approach that eliminates retraction and lessens nerve damage risk

When vascular surgeon Katherine Teter, M.D., recently joined Hackensack University Medical Center, she sought an unusual operating room partner—a thoracic surgeon with robotic surgery experience.
Geoffrey Pelz, M.D., was intrigued by her unique approach to rib resection, and the cross-specialty pair, both trained in robotic surgery techniques, recently performed the center’s first trans-thoracic robotic rib resection on the first rib. They have completed several additional rib resections since and are ramping up to a full case load.
Traditionally, first rib resection is an open surgery requiring retraction of critical blood vessels and nerves to access the underlying bone. The open approaches, especially transaxillary, is like “operating in a tunnel” with limited visibility near multiple vital structures versus a wide open highway, Dr. Pelz said.
By collaborating and handing off the robot controls mid-procedure, Dr. Teter and Dr. Pelz combine the benefits of the surgical robot’s precision and enhanced visualization with their individual areas of expertise to perform a safer, less disruptive procedure. Dr. Teter begins the robotic portion, exposing the rib and identifying the optimal points for division. She then moves to the bedside and performs the actual cutting of the rib. Dr. Pelz transitions from assisting to operating the robot, using his thoracic expertise to mobilize the surrounding soft tissue, muscles, and vessels off the rib. As needed, Dr. Teter returns to free up scar tissue around the vessels.
This team approach provides the greatest benefit to the patient, minimizing risk and speeding recovery. Routine cases may even be discharged the same day.
