Hackensack Meridian Health Expands Hospital From Home Program To Two More Hospitals, Expanding Access To This Successful Program Which Allows Patients to Receive Care In The Comfort Of Their Own Home

Five Hackensack Meridian Health hospitals now participating in Hospital from Home program that has treated more than 1,000 patients

Hackensack Meridian Health, New Jersey’s largest and most comprehensive health network continues to innovate how care is delivered and expand patient access to care with the expansion of Hospital From Home, a program that provides hospital-level care to patients from the comfort of their home.
Hospital From Home care will now be offered to patients who qualify through Ocean University Medical Center in Brick, New Jersey and Southern Ocean Medical Center in Stafford Township, New Jersey. The latest sites build on the network's robust existing Hospital From Home program currently offered through Hackensack University Medical Center in Hackensack, JFK University Medical Center in Edison, and Jersey Shore University Medical Center in Neptune, New Jersey.

Garrett, FACHE, chief executive officer of Hackensack Meridian Health. “For more than a year our Hospital From Home program has expanded access to care by allowing more than one-thousand patients to receive hospital level care in the comfort of their home. We are thrilled to expand this successful program to more patients in other communities we serve.”
Hospital From Home launched in late April 2024 after a successful 2022 pilot at JFK University Medical Center. Hospital From Home has cared for more than one-thousand patients from its sites at Hackensack University Medical Center, Jersey Shore University Medical Center and JFK University Medical Center. The network has plans to expand it to all hospitals across the health system.
Hackensack Meridian’s Hospital From Home program is enabled through a partnership with Medically Home, a DispatchHealth company, the national leader in hospital at home care.

Home hospital care is one of the fastest-growing modes of care, delivered through a hybrid model of virtual and in-person visits. Through Hospital From Home, patients are monitored virtually through a clinically integrated, physician-led virtual command center, which operates 24 hours a day to ensure seamless patient care alongside in-person nurse visits. Services and treatments include IV antibiotics, infusions, medication management, laboratory services, physical, occupational, speech and respiratory therapy and imaging in a patient’s home. Meals and nutritional consultations are also available.
Through Hospital From Home, patients like Harry Gordon, 70, received regular telehealth and in-person visits as part of her treatment for his urinary tract infection. “I had contact with my medical team with the push of a button,” Harry recounts. “The nurses came and took my vitals signs, gave me IV’s in my home, brought the medications I needed to my home, test results available quickly on MyChart. Everything worked seamlessly. I was able to do all this while at home and cooking the food I enjoy in my own kitchen. I would recommend this program.”
Clinical studies have shown home hospitalization improves patient outcomes, increases patient and clinician satisfaction, saves money, reduces mortality, and lowers the risk of readmissions.

The program was created under the Medicare CMS waiver during the COVID-19 pandemic and has since been granted an extension.
Medically Home collaborates with Hackensack Meridian Health as an operating partner, providing command center services, logistics, and technology needed to deliver hospital-level care in the home. Medically Home is participating in numerous successful partnerships with leading health systems nationwide.
Patients and families interested in this program should ask their admitting physician if they are eligible. Before starting care at home, individuals will receive an in-person physician evaluation and determine together if the program is right for them. Hackensack Meridian Health’s Hospital from Home accepts most major payors including Medicare, Cigna, Horizon Blue Cross Blue Shield of New Jersey, UnitedHealthcare and more.
