First-of-its-kind remote care delivery system will extend the reach of clinicians and future-proof care for New Jersey’s seniors
Hackensack Meridian Health, New Jersey’s largest and most comprehensive health network, and Cadence, the nation’s leading remote care delivery system, today announced a strategic partnership to build the infrastructure to deliver proactive senior care. By extending care beyond clinic walls through connected devices and Cadence’s AI-powered platform, Hackensack Meridian Health will help seniors stay healthier, more independent, and always connected to their care teams.
Through the partnership, Hackensack Meridian Health patients use connected devices at home to share vital signs such as blood pressure, weight, heart rate, and blood glucose. Cadence’s AI-powered Proactive Care Engine analyzes this data in real time, alerting the 24/7 clinical team when intervention is needed and ensuring patients receive support with medications, and lifestyle guidance.
