Our social determinants of health program, HMH Healthy Connections, is a three-pronged strategy designed to foster sustainable investments in underserved and marginalized populations through resources beyond traditional health care to meet their varied, whole-person needs. Its focus is on three primary areas:

  • Healthy Connections CHW Initiative - A community health worker-based intervention to address social determinants in targeted patient populations and communities using trained CHWs to provide direct support via in-person and virtual interactions.
  • Healthy Connections SDOH Initiative - A five-pronged anchoring strategy addressing food security, housing stability, transportation issues, caregiver support and mental health/behavioral health/SUD across the patient, VBC and geographic communities.
  • Healthy Connections SDOH Documentation Screener - A network-wide SDOH data collection effort focused on screening for and identifying SDOH needs across all patient Electronic Health Records
