HMH - Primary Care - Toms River - 1028 Hooper Avenue

Known to our patients as Toms River Family Medical Center, our practice has been caring for the Toms River community for more than 30 years.

We were recently recognized by the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home which means that we emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement.

Our practice has four locations: Freehold, Jackson, Sea Girt and Toms River, and some of the providers work in multiple locations.  

Appointments should be scheduled in advance. All of our locations welcome new patients.
