Sanzari Women’s Hospital at Hackensack University Medical Center and gynecology patients, who require surgery, will have their surgery at Hackensack University Medical Center.

To make your first visit as smooth and productive as possible, please bring the following items:
- Your photo identification and insurance card.
- A referral from your primary care physician, if required by your insurance plan.
- A list of all current medications, vitamins, and supplements you are taking, including dosages.
- Any recent lab results, blood sugar logs, or imaging reports (such as thyroid ultrasounds) related to your condition.
- A list of questions you have for the endocrinologist.
Our office is located at 20 Prospect Avenue, Suite 805, in Hackensack. We are open 8:00 a.m. - 4:00 p.m. Monday through Friday.
