Home Care Services
Our Home Care Services bring clinical expertise directly to patients in their homes. All services are provided by licensed professionals, ordered by a physician, and coordinated through a personalized care plan. Below is an overview of each discipline available through Hackensack Meridian's home health program.
Hospice Care
Hospice care is specialized comfort-focused care for patients and families facing a life-limiting illness when curative treatment is no longer the goal. Hospice is appropriate when a physician certifies that a patient has a prognosis of six months or less if the illness follows its expected course. The goal of hospice is to ensure the patient is as comfortable as possible while honoring their wishes and supporting their family.
Home Infusion Therapy
For patients who require intravenous (IV) medications but do not need to remain in a hospital or infusion center, Hackensack Meridian Health offers home infusion therapy services. Under the supervision of a registered nurse, medications are administered safely and effectively in the comfort of your home — reducing unnecessary exposure to healthcare settings and supporting a faster recovery.
Home infusion therapy may be ordered for patients receiving:
- IV antibiotics for serious infections
- Biologic therapies for conditions such as Crohn's disease, rheumatoid arthritis, or immune disorders
- IV hydration and nutritional support
- Chemotherapy support medications
- Post-surgical antibiotic therapy
Our infusion nurses are experienced in IV access management, patient education, and monitoring for medication reactions. Your care team coordinates directly with your prescribing physician to ensure the safest possible delivery of treatment at home.
Durable Medical Equipment (DME)
Durable Medical Equipment (DME) refers to medically necessary equipment prescribed by a physician to support recovery, manage a health condition, or assist with mobility and daily function at home. Hackensack Meridian Health can help coordinate the delivery of DME to ensure you have what you need before you leave the hospital or as your care needs change at home.
DME available through our program may include:
- Hospital beds and pressure-relief mattresses
- Wheelchairs and transport chairs
- Walkers, crutches, and canes
- Home oxygen equipment and concentrators
- Nebulizers for respiratory treatment
- CPAP and BiPAP machines for sleep apnea and respiratory conditions
- Wound care supplies
- Bathroom safety equipment including shower chairs and grab bars
DME must be prescribed by a physician and is generally covered by Medicare Part B and most private insurance plans when medical necessity is documented. Our team can assist with insurance verification and coordinating delivery to your home.
Getting Started: Three Simple Steps
- Ask your doctor for a referral. Your physician, hospital discharge team, or specialist can send a referral directly to our team. If you're unsure how to start, ask your care team about home health options at your next appointment or before hospital discharge.
- We'll contact you to schedule your evaluation. Once we receive the referral, a member of our team will reach out to schedule an initial assessment at your home. This visit allows our clinician to understand your needs and begin building your care plan.
- Your personalized care plan begins. Your care plan is built around your specific goals — whether that is recovering strength after surgery, managing a complex condition, or preparing for end-of-life care at home. Your team will communicate regularly with your physician and adjust the plan as your needs evolve.
