Accepted Student Checklist

Acceptance Instructions: Please read the instructions thoroughly as they must be followed completely.

1. Pay Acceptance Fee

Payment of the $150.00 non-refundable Acceptance Fee is due by the date listed on your acceptance letter.

Submit your Acceptance Fee by mail or drop off in person. Please send a check or money order payable to HMH JFK Muhlenberg Snyder Schools to:

Please NOTE: Acceptance Forms indicated below MUST accompany the payment.

2. Submit Acceptance Forms

Everyone, including students currently attending Union College, must complete and return each form listed below along with Acceptance Fee payment by the date listed on your acceptance letter.

Please print, complete, and return the PFD forms below.

It is important to send all forms by the date listed on the acceptance letter. Placement in any of the School’s courses is on a seat-availability basis.

3. Consider Financial Aid

If you are interested in applying for Financial Aid (grants, student loans, parent loans and scholarships), you must complete the FAFSA online at https://www.studentaid.gov. Learn more about financial aid. 

4. Complete Union College Application

Everyone must complete the online Union College application, except for those students currently attending Union College. Once the online Union College application has been completed, Hackensack Meridian Health JFK Muhlenberg Snyder Schools will process the change of major from Liberal Studies to Nursing, MHBN, Radiography, MRAY, or Sonography, MDMS major.

5. Submit Final Transcripts

A final transcript is required if you are recently taking courses and you did not submit those grades by the time you submitted an application. Official transcripts need to be submitted or a registration hold will be placed on your account . Send official transcripts to Adam.Klein@hmhn.org.
